Visit the NUTRILITE Facebook® page at www.Facebook.com/NutriliteUS and www.Facebook.com/NutriliteCanada and click on the Ultimate Football Experience tab to access the sweepstakes application, then complete the sweepstakes entry form with your name, email address, city, and state/province and include how you heard about the sweepstakes. You must be a fan of ("like") our fan page on Facebook to access the sweepstakes and Instant Win Game application. After submitting your sweepstakes entry, you will be re-directed to play the Instant Win Game. Choose one of the three boxes and use your mouse to click and scratch off the box to see if you are a potential winner, subject to verification. Limit one (1) entry and one (1) Instant Win Game play per person/email address per day.

Automated methods of entry/game play are prohibited and any use of automated devices will cause disqualification. In the event of a dispute regarding the identity of the entrant, the natural person associated with the email address provided with the entry will be considered the entrant. Participants may not enter with multiple email addresses or set up multiple Facebook accounts to enter multiple times, nor may entrants use any other device or artifice to register multiple times or as multiple registrants. Prizes:

Two (2) Sweepstakes Grand Prizes:

Each a trip for two to Phoenix, Arizona to attend the Kurt Warner: The Ultimate Football Experience fantasy day. Prize includes round-trip coach airfare to Phoenix, Arizona from a major airport nearest the winner's home; three (3) days/two (2) nights hotel accommodations (double occupancy); rental car; and (1) $500 Visa® Gift Card. Approximate retail value $4,500.00 USD each trip.

Winner and his/her Guest must travel together on the same itinerary and are solely responsible for obtaining all necessary travel documents, including passports, if necessary. At least one traveler must be 21 years of age or older. Only one person (either the Winner or the Winner's Guest) will be able to participate in the event's game(s). The participant must be at least 21 years of age or older and must sign any documents required by the organizer of the fantasy day to participate. If not participating in the event's games, Winner and/or guest will have access to the event's facilities.

All travel arrangements will be made through Sponsors' agent on a carrier of Sponsors' choice. Travel arrangements are subject to availability and Trip must be taken on the dates specified or trip shall be forfeited and Sponsor shall have no further obligation to such Winner. Sponsor is not responsible for any flight cancellations or postponement. All prize details are at Sponsor's discretion.

All other expenses not specified are the sole responsibility of the winner. Trip will be scheduled in March, 2013.

Odds of winning depend on the number of eligible entries received. Prizes are non-refundable and non-redeemable. All other expenses not specified are the sole responsibility of the winners. All taxes are the sole responsibility of the winner. No substitution or transfer of prize is permitted, except Sponsor reserves the right in its sole discretion to substitute the prize with a prize of equal or greater value.

Winner Selection:

Grand, First and Second Prize winners will be selected randomly by using a computer-generated application tool on or about January 14, 2013 from among all eligible entries received during the Entry Period. Instant Win prizes are randomly time-seeded throughout the Promotion Period and awarded to the first person who plays on or after the time associated with each prize. Odds of winning a sweepstakes prize depend on the number of eligible entries received. The time of day when you play the Instant Win Game during the Promotion Period (as recorded by the Web Site technology) and the number of entries received and the order in which they are received will solely determine whether or not you are a potential winner and your odds of winning. Daily winners will be eligible for the Grand, First and Second Prize drawing. Determination of winners will be the responsibility of Sponsor, whose decision will be final and binding in all matters related to this Sweepstakes.

Winner Notification:

Winners will be contacted via a traceable method of mail. Grand Prize winners and their guest must sign and return a standard Declaration/Affidavit and Liability/Publicity Release form. If any winner does not meet eligibility, fails to return a Declaration/Affidavit/Release within seven (7) calendar days after attempted notification, or does not comply with Sweepstakes rules, the prize will be forfeited and awarded to an alternate winner. In the event a prize winner is unable to redeem the prize due to reasons beyond their control no compensation or substitutions will be provided. First, second and Instant Win Game prizes will be shipped to the addresses used during registration approximately 4-6 weeks after the end of the Sweepstakes Period. Daily Instant Win Game prizes will be emailed to the winners throughout the promotion period. Unclaimed prizes will not be awarded.

For U.S. Grand Prize Winners: Grand Prize Winners must complete a W-9 Request for Taxpayer Identification Number and Certification form prior to gift being awarded if the market value is $600 or more. This is a requirement of the Internal Revenue Service for purposes of issuing a Form 1099-MISC for the fair market value of the prize awarded.

For Canadian winners: In order to be declared a winner, a time-limited skill-testing question must be answered without assistance of any kind. Any selected entrant who incorrectly answers the time-limited skill-testing question will be disqualified and, for Sweepstakes prizes, the prize may be awarded to an alternate entrant chosen by random draw who must then correctly answer the question (if the alternate selected is a resident of Canada). Instant win prizes will not be reseeded and will remain unawarded if a Canadian winner answers the time-limited skill-testing question incorrectly.

Winners agree that Sponsor shall have the right and permission to use winners' name, pictures, likeness and/or voice in any media for trade, advertising, promotional and/or other purpose in any media now or hereafter known without any further consideration except where prohibited by law.

Any litigation respecting the awarding of a prize may be submitted to the Régie only for the purpose of helping the parties reach a settlement.

By entering, entrants agree to be bound by these Official Rules and the decisions of the Sponsor, which shall be final and binding in all matters pertaining to the Sweepstakes. The Sponsor and its agencies, officers, directors, agents, and employees are not responsible for and shall not be liable for: (1) garbled, illegible, incomplete, damaged, misdirected, stolen, late, lost, or failed electronic entries; (2) telephone, electronic, hardware or software program, network, internet or computer malfunctions, failures or difficulties of any kind including any injury or damage to entrant's or any other person's computer related to or resulting from participating in or downloading any materials in the Sweepstakes; (3) errors or delays in transmission; (4) any condition caused by events beyond the control of the Sponsor which may cause the Sweepstakes to be disrupted or corrupted; (5) any losses or damages of any kind caused by the prize or resulting from acceptance or use of the prize, or from participation in the Sweepstakes, or (6) any printing or typographical errors in any materials associated with the Sweepstakes.
